  New Patients
  
  If you are interested in seeing Dr. Maier and if you have a primary care provider, Nurse Practitioner or Medical Doctor, please have them send us a written request for an appointment and authorize them to send us your medical history chart.
  
  If you want to transfer your rheumatology care from another rheumatologist to Dr. Maier, please call our main number and we arrange to continue your care.
  
  Please authorize a release of records from your current rheumatologist and fax them to our office at (541) 345-2821.
