• About The Practice Patrick Rhodes, MSN, APRN, FNP-C, is an experienced family nurse practitioner who leads the team at Supreme All Care Health and Wellness in Kingwood, Texas. Patrick has a desire to not only treat and care for patients but also to touch the heart of every person that he meets. Patrick and the board-certified team specialize in family medicine, with services that range from annual physicals and weight loss management to IV hydration therapy and allergy testing. They are trained in the latest technologies and procedures to ensure optimized health outcomes for all of their patients. Patrick was raised in the small town of Crockett, Texas. He completed his bachelor’s degree in nursing at The University of Texas at Tyler. He went to complete his Master of Science in Nursing at Chamberlain University in 2019, specializing as a Family Nurse Practitioner. He is a member of and board certified through the American Academy of Nurse Practitioners. Patrick Rhodes works under the medical direction of Flavia Thomas, DO who has over 20 years of medical experience and expertise to ensure the needs of all patients are met with top quality care by our team.
