MyCareCoach is an integrative, evidence-based chronic care program that is designed to help patients with two or more chronic conditions. We will provide the bridge in that gap of healthcare that happens in the office. Our CareCoach team is a group of highly qualified, compassionate, and respectful professionals covered under Medicare insurance who will help you and/or your loved one: *Better understand your conditions and treatment plan laid out by providers *Ensure quality care by coordinating between all providers *Develop skills that will prevent relapse *Build attainable health goals and document them for you and all providers *Guide you carefully through early stages of learning how to self-manage your conditions *Learn how to interact better with providers during short visits We want to create an opportunity for you to get more out of your healthcare experience and live a healthier life by helping you become an active part of your health care team.
