About Us Autumn Country Club Adult Day Care provides a safe, structured, and long term care program for families attempting to keep their loved one in their home where they are loved and thrive. Autumn provides an active, social, stimulating day for our members and provides much needed respite for their caregivers. Autumn's registered nurses monitor the health of our members daily. Autumn's aides provide activities such as crafting, games, exercise programs, and music. Autumn's CNAs provide washroom assistance and personal care.
